As a student social worker I did the following • To provide a person-centred and strengths-based approach, which adheres with the principles of the Care Act 2014. • To provide advice, information, needs based assessments, care reviews, Continuing Healthcare referrals and safeguarding management - as per SET (Southend, Essex, Thurrock) guidelines. • Running in conjunction with intervention, the Mental Capacity Act 2005 is considered and implemented when appropriate and necessary. • Provide support based on the evidence of their assessments and this can range from commissioned services, advice, signposting and direct payments - services are requested by the social worker, and budget holders determine, based on the social workers assessment, whether funding is authorised or not. • Reviewed all care implemented within 6 weeks and further intervention is determined/finalised once the review is completed.
